A top surgeon has weighed in on Simon Cowell‘s ever changing face after the music mogul turned 65 on Monday.
In recent years, Simon’s appearance has become just as much of a talking point as his hit TV shows – with fans commenting on his radical transformation.
In 2019, Simon admitted to having a ’21st-century face lift’ and ‘too much Botox‘ before his 60th birthday.
He gave himself a huge makeover and said the procedure ‘hurt like hell’ as he revealed other methods he has turned to, before dialling it back.
As Simon turns 65, Professor Ullas Raghavan, a renowned cosmetic surgeon at Pall Mall Medical, has given his view of Simon’s age-defying visage.

In recent years, Simon’s appearance has become just as much of a talking point as his hit TV shows – with fans commenting on his radical transformation (pictured in August)
In the 1980s, Cowell was far from the polished, suited TV mogul recognised today.
Working behind the scenes in the music industry, he sported a youthful, natural appearance with a fuller face, un-styled hair and his own teeth.
By the early 2000s, when Pop Idol and American Idol catapulted him to fame, his style had sharpened.
Simon was known for his casual wardrobe of black T-shirts and jeans, but his face still showed the hallmarks of natural ageing, with subtle lines and a softer jawline.
Fast forward to the 2010s, and his look started to change dramatically. Simon’s face appeared more sculpted, with fuller cheeks and a smoother forehead.
This transformation sparked a wave of speculation about the cosmetic interventions he might have undergone.
Professor Raghavan suggests Simon could have opted for several procedures to maintain his youthful appearance.
‘From what we’ve seen over the years, Simon may have had Botox injections to smooth out wrinkles on his forehead and around his eyes,’ Prof Raghavan told MailOnline.

‘The fuller cheeks could suggest dermal fillers to restore volume that naturally decreases with age. This would give his face a more youthful, plumped appearance, counteracting the natural hollowing that occurs over time.’
Professor Raghavan also speculates the Britain’s Got Talent judge may have undergone additional cosmetic procedures to enhance his appearance.
‘He may have had a facelift to tighten the sagging skin around his jawline, giving him a more defined and sculpted look. I wouldn’t be surprised if he’s also had a face and neck lift, as well as a brow lift, which would elevate his brows and open up his eyes, giving him a more refreshed and alert appearance.
‘A blepharoplasty (eyelid surgery) would likely have helped rejuvenate the area around his eyes, removing excess skin or fat from the upper or lower eyelids. This would smooth out any puffiness or drooping and further contribute to the youthful transformation we’ve seen in Simon over the years.’

‘When facial injectables are overused, they can reduce mobility and create a ‘stiff’ look, especially around the eyes and mouth’, said Prof Raghavan

Pictures taken throughout the years suggest Simon started having Botox injections from around 2009 onwards.
In 2019, Simon admitted to having the facelift – but didn’t say when, and said the procedure ‘hurt like hell.’
He told The Sun that he visited cosmetic surgeon Dr Jean-Louis Sebagh, known as the ‘Picasso of dermatology’, for a Silhouette Soft Lift.
He underwent a non-surgical ‘sewing bioplastic-infused thread’ facelift that set him back £2,000.
Simon explained: ‘For me now, it’s all about having clean skin. If you have clean skin, you look better. But you have to be very careful with some of this stuff.’
‘There’s lots of things you can do now. You don’t just have to stuff your face with filler and Botox. It hurts like hell but it gets rid of sun damage and unplugs all the cr*p.’
In 2019, Simon lost 20-pounds after adopting a vegan diet, and embracing a healthier lifestyle. While his physique became leaner, his face became a talking point amongst fans who asked: ‘What on earth has happened to your face?’.
‘When facial injectables are overused, they can reduce mobility and create a ‘stiff’ look, especially around the eyes and mouth, which is something we’ve noticed with Simon in recent years’, said Prof Raghavan.
Simon once claimed that using Botox ‘added 10 years’ to his TV career, but that he regrets going overboard with the procedure.
He said: ‘Hopefully I look better now – I probably did have a little too much Botox a couple of years ago, because everyone on TV has it.
‘Now I have facials, but nothing too extravagant at the moment. I have a girl who comes here and does a great face massage – it’s very relaxing. I also had a sheep placenta facial once, which was quite out there.’
Now, as he steps into his mid-60s, Simons face has taken on a more natural, yet still youthful, appearance.
Professor Raghavan speculates that Simon might be maintaining his look through more subtle, regular non-invasive treatments, such as laser skin resurfacing and skin-tightening procedures.
‘He seems to have found a balance where he’s still addressing the signs of ageing but not overdoing it’, he said.
‘As Simon Cowell marks his milestone 65th birthday, his transformation over the years reflects not only the pressures of staying youthful in the entertainment industry but also the fine line between enhancement and excess.’
MailOnline has contacted Simon’s representatives for comment.
